Course Information

This course explains the SWAN model for end-of-life and bereavement care, which stands for Signs, Words, Actions, Needs. It provides an overview of how the SWAN model was developed and what its aims are in caring for people and families when someone is dying or has died. 

You will learn when and how to apply the SWAN principles in a care home or hospital setting – such as recognising when the model should be used, and how to implement its compassionate practices to support dying individuals and their families. The course shows how SWAN can be adapted for use in care homes to promote dignity and comfort at end of life.
